What are the objectives of the survey?
If you are 18 years or older and have tested positive for COVID-19, we invite you to participate in a survey study to help us understand the impact of the disease on your ability to be physically active.
The Nicholas Institute of Sports Medicine and Athletic Trauma (NISMAT) at Northwell Health is conducting an online survey to measure physical activity of those who have had COVID-19. By doing so we will gain important knowledge about how long people are taking to return to their former level of activity, and if prior activity levels help people recover more quickly.
Who can participate?
Only adults over 18 years old who have tested positive for COVID-19 can participate. The testing must have been done by nasal swab and only adults who recovered without needing hospitalization can participate. Participation is voluntary.
How long does the survey take?
It will take approximately 10 minutes each time you complete the survey. First, we will ask you to estimate your physical activity in the 3-month period just before you were diagnosed with COVID-19 using a 13-question survey, and then you will repeat the survey each month thereafter. Please try to answer all questions; do not skip questions or sections and be as honest as possible. In this way we can see how long it is taking people, on average, to return to the level of physical activity they had enjoyed prior to diagnosis.
How does it work?
Please answer all questions. The survey can be interrupted and come back to at a later time. Once you have answered, information from your submitted survey cannot be withdrawn. We are not collecting anyone’s names.
The first time you take the survey we will ask you general questions such as age, and height and weight, and then ask you to think back to 3 months prior to being diagnosed and how much physical activity you performed during those months of your “regular” life before the COVID-19 pandemic. You will answer 13 questions regarding that activity. We will ask you to also answer questions regarding your activity in the past month, after having been diagnosed with COVID-19. We will ask you to retake that 13-question activity survey each month thereafter, describing how active you have been in that month. We anticipate collecting surveys for at least one year, but if a significant portion of the participants are reporting that they are not recovering a year after diagnosis we will invite you to continue participating up to one year more.
How will my personal information and I be protected?
If you agree to be in this study, you are giving us permission to collect, use, and share your health information provided in your survey responses. If you have any question on the survey please contact the project leads, Dr. Susan Kwiecien at SKwiecien@Northwell.edu or Beth Glace at BGlace@Northwell.edu. There is no risk related to the survey, except that you could become more aware of the impact that the COVID-19 pandemic may be having on your physical well-being.
Study records that identify you will be kept private.
There is no limit on the length of time we will keep your survey responses for this research because it may be analyzed for many years.
By agreeing to participate in this research, you have not compromised any legal rights. All information from your participation in this research will be collected and stored in accordance with the Health Insurance Portability and Accountability Act of 1996. By agreeing to this study, you are agreeing to Northwell Health processing personal data that are not linked to you personally and that you provide for any purpose connected to this study.
